These Terms of Use govern your use of cemlif.com (the “Site”), operated by the practice of Dr. Mark Frenkel, 6101 Pine Ridge Road, Naples, Florida 34119 (“we,” “us,” or “the practice”). By using the Site you agree to these Terms. If you do not agree, please do not use the Site.
The content on this Site — including articles, FAQs, videos, and descriptions of the CemLIF procedure — is provided for general educational purposes only. It is not medical advice, does not describe every risk or alternative, and is not a substitute for an in-person evaluation by a qualified physician who knows your medical history. Individual results vary. Please read our Medical Disclaimer.
Using this Site, reading its content, submitting a form, or uploading imaging does not create a physician–patient relationship between you and Dr. Frenkel or the practice. A physician–patient relationship is established only when the practice formally accepts you as a patient.
Do not use this Site, its forms, or its upload tool for urgent or emergency medical needs. Submissions are reviewed during normal business hours and are not monitored continuously. If you think you may have a medical emergency, call 911 or go to the nearest emergency room immediately.
The Free MRI Review is a complimentary, preliminary look at imaging and information you choose to send us, offered to help you understand whether you might be a candidate for the CemLIF procedure. It is not a diagnosis, a treatment recommendation, a second opinion, or a guarantee that you are a candidate or that any treatment will be offered or covered by insurance. Requesting a review creates no obligation on your part or ours. You are responsible for the accuracy of the information you submit and for having the right to share any imaging or records you send us.
Information you submit is handled as described in our Privacy Policy. Do not submit passwords or account credentials through the Site. You agree not to submit content that is unlawful, misleading, or that you do not have the right to share.
Patient stories, testimonials, and reviews on this Site reflect the individual experiences of those patients. They are not a promise or prediction of your results, which depend on your condition, health, and many other factors.
The Site and its content, including text, graphics, logos, images, and video, are owned by or licensed to the practice and protected by copyright and trademark law. “CemLIF” is a trademark of the practice. You may view and print pages for your personal, non-commercial use. Any other use requires our prior written permission.
The Site links to or embeds content from third parties (for example YouTube, a reviews widget, and the image-sharing service mymedicalimages.com). We do not control those services and are not responsible for their content, availability, or privacy practices. Your use of them is governed by their own terms.
The Site is provided “as is” and “as available.” To the fullest extent permitted by law, we disclaim all warranties, express or implied, including warranties of accuracy, completeness, merchantability, fitness for a particular purpose, and non-infringement. Nothing in these Terms limits liability that cannot be limited under applicable law, including for professional medical services actually rendered to you as a patient.
These Terms are governed by the laws of the State of Florida, without regard to its conflict-of-law rules. Any dispute arising from these Terms or your use of the Site will be brought in the state or federal courts located in Collier County, Florida, and you consent to their jurisdiction.
We may update these Terms from time to time. The date at the top of this page reflects the most recent update. Continued use of the Site after a change means you accept the updated Terms.
